Invicta HouseCall is a transitional care service for Medicare Homebound clients.
We provide comprehensive physical assessment, Face to Face, medication reconciliation, and medication refills to our Medicare Homebound clients. Other services include ordering Durable Medical Equipment (DME) – wheelchair, walker, hospital bed, bedside commode, and so on.
Patient referral to appropriate medical specialists, within our network, including but not limited to – Neuro., Ortho., Pain Mgmt, Psychiatrist, Podiatrist, Vascular. All our medical specialist accept Medicare and or Medicaid.
Tests include Echocardiogram, Peripheral Vascular Profile(venous and arterials), Carotid, Thyroid, and Abdominal Scan.
